A high-profile charity review into complaints about a social care home has called for fundamental changes to the standards of Britain's health and social care system.
The report, compiled by independent expert Margaret Flynn, criticises health regulators, local health services, police and the owners of Winterbourne View for failing to act on warning signs of abuse by staff at the care home.
Public Health Minister Anne Milton said the independent case review, which contained 260 complaints from relatives and carers, showed "failure at every level".
She told BBC News there was evidence the home's commissioners did not have "any sort of monitoring in place".
Labour's Shadow Minister for Care and Older People Liz Kendall called on the Government to "spell out" what measures they will take to prevent similar abuse happening in future.
"With many hundreds of vulnerable people with learning disabilities living in large institutions similar to Winterbourne View, the Government must now spell out how abuse like this will be prevented from happening again," she said.
Mencap and The Challenging Behaviour Foundation say the abuse of vulnerable patients is at risk of being repeated if changes are not made. The abuse exposed by BBC Panorama includes allegations of neglect and sexual assault.
